Employee Assistance Program (EAP)

Live and Work Well is an Employee Assistance Program (EAP) designed to provide confidential support for challenges that employees face in their everyday lives.

Services include, but are not limited to:

    • Mental health
    • Substance abuse
    • Workplace problems or conflicts
    • Work/Life balance
    • Grief and loss
    • Coping with change
    • Eating disorders
  • Financial and legal assistance
  • Family support
    • Parenting and family issues
  • Help with relationships
  • Health and wellness support
  • Referrals to community resources
  • Assistance in choosing the best providers


All records, including medical information, referrals and evaluations are kept strictly confidential in accordance with federal and state laws.


Counseling services are available at no cost to you, your dependents, and anyone residing in your household. Each eligible member has five (5) counseling
sessions, per issue, per year. If additional services are recommended, the counselor will assist you with accessing providers covered by your insurance plan.

Counseling services approved by OptumHealth will no longer be available through OptumHealth effective 07/31/2021 .
Employees and/or family members should contact LifeMatters to initiate approval of additional services.

Contact information starting 8/1/2021: Call 1-800-367-7474, 24 hours /day, every day of the year to receive LifeMatters EAP services. Additional access to Life Matters is available through their website, www.mylifematters.com. Enter our company password csuci to obtain educational information, useful links, help locating child and elder care providers, self-assessment tests and much more!
